Clinical Supervision Guides›Free Supervision Toolkit›Supervision Log Template
Last updated: November 4, 2026
A clinical supervision log is the supervisor's running record of every supervision session with each supervisee, kept alongside the dates that matter over the whole supervisory relationship. The template below has two parts, a key-dates box for each supervisee and a session log with one row per session, and it comes as a printable PDF and as a spreadsheet that totals each week for you.
The log is part of the Sentio MFT program's free clinical supervision toolkit.
This is an optional aid from the Sentio MFT program's clinical supervision toolkit. This is not a BBS form.

Is There a Spreadsheet Version of the Supervision Log?
Yes. The supervision log spreadsheet keeps the same record in Google Sheets and adds up each week for you. Open it, choose File and then Make a copy, and use the copy. It has four tabs:
- Supervisees holds the key-dates box for each supervisee, one row per person, with the deadlines filled in from the start date.
- Session log takes one row per supervision session: date, supervisee, setting, format, mode and hours.
- Weekly summary adds the sessions into the figures you check before you sign each supervisee's weekly log: individual or triadic hours and group hours, the units of direct supervisor contact, and a flag for any week that falls short of the weekly rule or goes over the six-hour limit.
- Read me explains each column and the rules behind the checks.
The weekly summary uses the same split as the Board's weekly log for marriage and family therapist trainees and associates, form 37A-525, which records individual or triadic supervision and group supervision separately. Your supervisee can keep their own hours in the California AMFT Hours Tracker, a companion sheet that totals their hours against each licensure requirement. The two sheets use the same week-starting date, so a week can be checked line by line before you sign.
What Supervision Records Does California Require?
The supervisee keeps a weekly log of all hours of experience, signed by the supervisor every week, and retains the signed logs until licensed; there is a separate log for each supervisor and each work setting, and the Board can ask to see them (16 CCR 1833(d)). For marriage and family therapist trainees and associates the current form is the Weekly Log of Experience Hours, 37A-525. When supervision ends, the supervisor signs an Experience Verification for the hours completed, with separate forms for hours before and after the degree.
The weekly rules decide what you are signing for. An associate needs at least one unit of direct supervisor contact in each week, in each work setting where hours are claimed, and an additional unit in any week the associate provides more than 10 hours of direct clinical counseling in that setting. A trainee needs at least one unit in each week in each setting, plus an average of at least one unit for every five hours of direct clinical counseling each week in each setting. One unit is one hour of individual or triadic supervision or two hours of group supervision, and no more than six hours of supervision can be credited in a single week (BPC 4980.43.2). Our guide to BBS supervision requirements for associates covers the rest.
Your own log is not a Board form and nothing requires it. It is the record that lets you sign each weekly log with confidence, answer a Board question months later, and keep track of the deadlines in the key-dates box. The BBS supervision forms guide explains each form named there, and our Clinical Supervision Guides cover the videoconference assessment.
Note on the videoconference assessment: through December 31, 2026, California requires it for every supervisee within 60 days of the start of supervision. From January 1, 2027, it is required only when supervision is conducted by videoconference, within 60 days of the start of that supervision (SB 1445). The videoconference assessment record covers each factor the law names.

Frequently Asked Questions
What is a clinical supervision log?
A running record, kept by the supervisor, of every supervision session with each supervisee: the date, format, mode, length, work setting, any recording reviewed and whether the weekly log was signed, together with the key dates of the supervisory relationship.
Is the supervisor's log the same as the BBS weekly log?
No. The weekly log is the supervisee's Board form, which you sign every week. Your log is your own record of the supervision you gave, and it helps you check each week before signing.
How many units of supervision does an associate need each week?
At least one unit in each week, in each work setting where hours are claimed, plus an additional unit in any week with more than 10 hours of direct clinical counseling in that setting. One unit is one hour of individual or triadic supervision or two hours of group supervision.
Can the spreadsheet replace the Board's forms?
No. It is a checking and record-keeping aid. The Board receives the Experience Verification you sign when supervision ends, and the supervisee keeps the signed weekly logs.
